Chronic fungal sinusitis with mycetoma

Findings:
T2 hypointense mass, representing fungal material with high proteinaceous content, in the right antrum with surrounding polypoid mucosal thickeneing, expanding the antrum into the right nasal cavity.
Mild polypoid mucosal thickening is also noted in the left maxillary sinus and in ethmoid air cells bilaterally. The paranasal sinuses are otherwise clear.
Small polypoid soft tissue lesions are seen within the middle nasal turbinates bilaterally, measuring approximately 8 x 12 mm on the right and 5 mm on the left. Consequential partial obstruction of the right nasal cavity. Mild nasal septal deviation to the left is noted.
Prominent nasal pharyngeal adenoid is seen.
The orbits are clear.
The imaged brain parenchyma is unremarkable.

Conclusion:
Features compatible with right-sided maxillary sinus mycetoma expanding the antrum into the right nasal cavity.
Also, mild sinonasal polyposis with partial blockage of the right nasal cavity.
