Whole spine, degeneration

Findings:
The craniocervical junction is normal. The cord and conus are unremarkable. The cord terminates at L1.

Modic-1/2 changes at C6/7 with bandlike bone marrow oedema.
Moderate to high-grade disc degeneration is seen in the cervical spine with decreased height and signal intensity of the intervertebral discs.
Predominantly right-sided osteoarthritis of the facet joints is seen at C2/3 and C3/4 with moderate neural exit foraminal stenosis at C2/3 on the right side, high grade also at C3/4 on the right side.
High-grade osteoarthritis of the facet joints is also seen at C4/5, more pronounced on the right side. Foraminal impingement of the right-sided C5 nerve root and moderate to high-grade neural exit foraminal stenosis at this level on the left side.
Unchanged high-grade osteoarthritis of the left-sided facet joint at C5/6 with foraminal impingement of the left-sided C6 nerve root and moderate to high-grade neural exit foraminal stenosis at this level on the right side.
Broad-based disc bulging and bilateral osteoarthritis of the facet joints and uncovertebral joints is seen at C6/7 with high-grade bilateral neural exit foraminal stenosis at this level, more pronounced on the left side.

Mild Modic-1 endplate changes are seen at T9/10 and T10/11 with associated disc degeneration and broad-based disc bulging at these levels without any relevant stenosis.

Scoliotic deformity of the lumbar spine is seen with its convexity to the left. Consequential loss of physiological lumbar lordosis is seen.
Known, grade 1, degenerative anterolisthesis at L4/5 and posterolisthesis at L1/2, L2/3 and L5/S1 is seen again.
To the previous study of November 2017 marked increase in the Modic-1/2 changes at the L5/S1 on the left side. Additionally, mild bone marrow oedema is seen involving the left-sided facet joint at L4/5 and L5/S1, in keeping with facet joint arthropathy with active synovial inflammation.
Minimal, newly appreciated bone marrow oedema is also noted involving the posterior aspect of the endplates at L1/2 and L2/3.
Unchanged, broad-based disc bulging and bilateral osteoarthritis of the facet joints at L1/2 -L3/4 without any significant stenoses.
Broad-based pseudo-disc and bilateral osteoarthritis of facet joints is also seen at L4/5 and L5/S1 without any nerve root impingement.

Partially imaged sigmoid diverticulosis. No paravertebral abnormality is identified.

Conclusion:
To the previous study of July 2019 progressive Modic-1 endplate changes at C6/7.
Unchanged osteoarthritis of the facet joints in the cervical spine with high-grade neural exit foraminal stenosis at right-sided C3/4 and C4/5 as well as at C5/6 and C6/7 bilaterally.
Mild Modic-1 endplate changes at T9/10 and T10/11.
Modic-1 endplate changes at L5/S1, minimally also at L1/2 and L2/3.
Facet joint arthropathy with active synovial inflammation at L4/5 and L5/S1 on the left.
