Findings:
Fluid intensity is seen extending from the left frontal subarachnoid space into the ipsilateral anterior ethmoid air cell.
No intracranial mass lesion or collection is identified. There is no hydrocephalus or midline shift.
A few non-specific foci of clear hyperintensity are noted in bilateral cerebellar white matter. The brain parenchyma is otherwise unremarkable.
The sella is enlarged, containing fluid of CSF signal. The pituitary is flattened to the floor of the sella. The infundibulum traverses the sella, without displacement. No fluid is noted within the sphenoid sinuses.
The orbits and mastoid air cells are clear.
The craniocervical junction is normal.
Conclusion:
MRI features suggestive of a CSF rhinorrhea from a bony defect of the left cribriform plate.
