Arteriovenous malformation – cerebral

Findings:

Serpiginous flow-voids at the right superior temporal gyrus, which represents the nidus of arteriovenous malformation. The nidus measures 2.2 cm in its larger diameter and is not associated with surrounding brain parenchyma signal abnormalities.

Arterial supply is arising from the right middle cerebral artery, which the MCA itself is larger in caliber compared to left counterpart.

Multiple sites of draining veins, include dilated right vein of Trolard, right cortical veins draining into the superior sagittal sinus, another draining vein is via enlarged right vein of Labbe into the right transverse sinus.

No arterial aneurysm.

On the SWI foci of susceptibility artefact are noted in the nidus of the AVM, likely in keeping with phleboliths. No obvious haemorrhage.

The brain parenchyma is unremarkable without evidence of any focal oedema or diffusion restriction.

The ventricles and sulcal spaces are within normal limits. There is no hydrocephalus or midline shift.

The pituitary gland is normal.

The orbits, paranasal sinuses and mastoid air cells are clear. 

No bony abnormality is seen. The craniocervical junction is normal.

Impression:

Right temporal arteriovenous malformation, Spetzler-Martin grade 1.
