Hyperparathyroidism – parathyroid adenoma

Findings:
There is a well-defined heterogeneously and avidly enhancing soft tissue mass lesion in the right upper paratracheal region, posteroinferior to the right thyroid lobe measuring approximately 18 x 12 x 26 mm. No calcification seen.
Several, small nodules can be observed in both thyroid lobes with no significant enlargement of the thyroid gland.

The soft tissue of the neck is otherwise unremarkable. There is no lymphadenopathy.
The cervical vessels are patent with no stenosis or occlusion.
The visualised brain is unremarkable. The paranasal sinuses and the mastoid air cells are clear.
Mild spondylodegenerative changes of the cervical spine are noted. No suspicious bony lesions.

Impression:
Features likely suggest primary hyperparathyroidism due to a right inferior parathyroid adenoma in the right upper paratracheal region. Correlation with 99mTc-Sestamibi SPECT/CT scan is advised.
