Trigeminal schwannoma.

Findings:
A vividly-enhancing predominantly homogenous (T1 hypointense / mildly T2 hyperintense with moderate diffusion restriction) ovoid mass is seen, centered in the right temporal pole, extending into the anterior aspect of the right Meckle’s cave. Further enhancement is also seen in the proximal segment of the right V2-nerve extending towards the foramen rotundum.
Possible dural tail sign is noted in the anterior and posterior aspect of the mass lesion.
Mild associated perifocal oedema in the right temporal lobe. No significant mass effect or midline shift.
The cavernous sinuses and the sella are unremarkable.
Moderate background small vessel ischaemic disease. The brain parenchyma is otherwise unremarkable.
Minimal fluid is seen in a few mastoid air cells on the right side. The paranasal sinuses and mastoid air cells are otherwise clear.
Bilateral IOL-implants are seen.
No bony abnormality. The craniocervical junction is normal.

Conclusion:
Intracranial extra-axial mass lesion in the right middle cranial fossa, extending into the right Meckle’s cave and along the right V2 nerve towards the foramen rotundum.
It is uncertain whether this represents a trigeminal (V2) Schwannoma or meningioma, with the former slightly favored.
