Findings:
Small well-defined extra-axial broad-based mass In the right cerebellopontine angle, overlying the right posterior aspect of the clivus, measuring approximately 13 x 7.5 x 13.5 mm (ML x AP x CC). The lesion is located superior to the right Meckel’s cave and shows broad base contact with the anterior aspect of the right tentorium cerebelli.
It elicits isosignal to the cortical grey matter on both T1, T2, and FLAIR with moderate diffusion restriction and vivid homogeneous enhancement on postcontrast sequences. No significant mass effect on the adjacent cisternal segment of the trigeminal nerve or the pons.
Enhancing lesion is also noted located in the left half of the sella, which cannot be separated from the pituitary gland, measuring approximately 8 x 6 mm. The pituitary stalk is central in position and normal in thickness.
No further intracranial mass lesions could be identified.
The ventricles and sulcal spaces are within normal limits. No midline shift. The brain parenchyma is unremarkable.
Mucous retention cyst is seen in the left maxillary sinus. The orbits, paranasal sinuses and mastoid air cells are otherwise clear.
No bony abnormality is seen. The craniocervical junction is normal.
Conclusion:
Small right cerebellopontine angle meningioma without mass effect.
Possible sellar mass lesion located in the left half of the sella, DDX small sellar meningioma, DDX pituitary microadenoma. Further evaluation with MRI pituitary gland is advised.
