Craniofacial fibrous dysplasia

Findings:
Very prominent changes (bone expansion with intact cortex and ground glass matrix) consistent with known fibrous dysplasia are demonstrated throughout the skull base bilaterally, involving the right frontal bone, the left maxilla and the left upper alveolar ridge. The mandible is also involved on the left.
Cranially the bony expansion extends to the both mastoid and occipital bones.
The body of the sphenoid bone as well as greater and lesser wings are markedly expanded with obliteration of the sphenoid sinuses.
Narrowing of the foramen ovale is seen bilaterally.
Narrowing of the right inferior and superior orbital fissures. No obvious narrowing of the optic nerve canals.

Previous left sided canal wall down mastoidectomy with smooth soft tissue surrounding the resection cavity. Obliteration of the remaining mastoid air cells on the left.
Loss of the bony plate between the right jugular bulb and the middle ear cavity on the right with a small jugular bulb diverticulum.
The mastoid air cells on the right and the paranasal sinuses are otherwise clear.

Conclusion:
Typical appearances of extensive craniofacial fibrous dysplasia.

Dehiscent jugular bulb on the right.
