Dentigerous cyst

Findings:

Cystic lesion is seen related to an inverted and mesioangularly ectopic /unerupted left upper 3rd molar tooth seen projected within the left maxillary antrum with associated cystic lesion surrounded by ill-defined calcific density. This presumed dentigerous cyst is unilocular and measures approximately 20 x 45 x 22 mm. The bony inferior margin of the cyst appears to communicate with the 27 and 28 root cavities. The margins of the cyst is thick walled with some areas of dehiscence.
Deep dental caries is seen involving the left upper second molar.

Mesioangularly uninterrupted right third molar tooth with as well as horizontally impacted lower third molars no associated cystic lesions.

Complete opacification of the left maxillary sinus is seen with mucoperiosteal thickening, expansion of the ostium into the left nasal cavity and obliteration of the left osteomeatal complex.

Also, subtotal opacification and mucoperiosteal thickening is seen in the left frontal and ethmoid sinuses. Mild mucosal thickening is seen in the right frontal, ethmoid and maxillary sinuses as well as in both sphenoid sinuses. The osteomeatal complex on the right is also obliterated.

Concha bullosa is seen on the right side with mild nasal septal deviation to the left.

The mastoid air cells and the middle ear cavities are clear.

Conclusion:
Appearances in keeping with a left dentigerous cyst extending into and involving most of the left maxillary antrum.
Impacted right upper third molar and both lower third molars without associated cystic changes.
Chronic pansinusitis with obliteration of the osteomeatal complexes bilaterally.
