Findings:
A large, solitary, lucent, unilocular, expansile lesion with smooth, corticated borders is seen In the maxilla which occupies the majority of the left maxillary sinus. The lesion shows internal soft tissue density and partial surrounding sclerosis. It measures approximately 3 x 4.5 x 4 cm. Laterally the lesion erodes the lateral wall of the left maxillary sinus and expands into the left masticator space.
Mucosal thickening is seen in the left maxillary sinus. Minimal mucosal thickening is seen also on the floor of the right maxillary sinus. The paranasal sinuses are otherwise clear.
Periapical cyst at 22.
The mandible is unremarkable.
Multiple, primarily non-specific bilateral level II and III lymph nodes are seen.
Conclusion:
Expansile, unilocular lesion of the left maxilla, extending into the left maxillary sinus and the left masticator space, likely in keeping with an odontogenic keratocyst with soft tissue involvement.
