Second branchial cleft cyst type IV

Findings:
A well-defined, lobulated cystic lesion is noted located in the left parapharyngeal space, indenting the left aspect of the oropharynx posterior to the left palatine tonsil. The lesion measures approximately 2.7 x 1.2 x 3 cm. Consequential mild narrowing of the pharyngeal airway. No invasion of the surrounding structures and no solid components.
Parotid and submandibular glands are unremarkable. There is no significant lymphadenopathy.
Minimal mucosal thickening is seen in the maxillary sinuses bilaterally, more pronounced on the left. The paranasal sinuses and mastoid air cells are otherwise clear. The nasal cavities are unremarkable.
The visualised brain is unremarkable. No hydrocephalus or midline shift.
No bony abnormality is seen.

Impression:
Left para pharyngeal cyst, medially indenting the oropharyngeal wall, most likely in keeping with second branchial cleft cyst type IV. No sinister pathology.
