Alzheimer disease

Findings:
No intra- or extra-axial mass lesion or collection is identified. The midline structures are normal with no midline shift.
No sign of atrophy of the midbrain or the cerebellum.
Global cortical atrophy score (Pasquier scale): 13 – mild opening of sulci, mild ventricular enlargement.
Koedam score for parietal atrophy: 1 – mild sulcal widening, no gyral atrophy.
Mesial Temporal Atrophy: 2 – moderate widening of the choroid fissure, mild enlargement of the temporal horn and mild loss of hippocampal height.
No significant small vessel ischaemic disease.
The orbits have a normal appearance and the paranasal sinuses and mastoid air cells are clear.
No bony abnormality is seen.

Conclusion:
Mildly prominent bilateral mesiotemporal atrophy, suspicious of mild Alzheimer disease or limbic-predominant age-related TDP-43 encephalopathy (LATE).
