Optic perineuritis (OPN)

Findings:
Subtle circumferential perineural enhancement of bilateral optic nerves is seen. The optic sheaths are mildly prominent. No evidence of any retrobulbar mass lesion.
No intra- or extra-axial mass lesion or collection is identified. The ventricles and sulcal spaces are within normal limits. There is no midline shift.
The brain parenchyma is unremarkable. No evidence of any enhancing intra or extra-axial mass lesion. No diffusion restriction.
The intracranial circulation is unremarkable.
Empty sella is noted again.
Small mucous retention cysts are noted in bilateral maxillary sinuses. The paranasal sinuses are otherwise clear.

No bony abnormality is seen. The craniocervical junction is normal.

Conclusion:
Subtle perineural enhancement of bilateral optic nerves, likely in keeping with optic perineuritis.
Empty sella.
