Findings:
Head:
No intra- or extra-axial mass lesion or collection is identified. The ventricles and sulcal spaces are within normal limits. The midline structures are normal with no midline shift.
Multiple, non-specific foci of T2/FLAIR hyperintensity are noted in bilateral cerebral white matter.
The brain parenchyma is otherwise unremarkable without evidence of demyelinating lesions. No abnormal signal on the susceptibility-weighted images or diffusion/ADC images is identified.
The pituitary gland is normal.
The orbits, paranasal sinuses and mastoid air cells are clear.
No bony abnormality is seen.
Spine:
The craniocervical junction is normal. The cord and conus are unremarkable without evidence of demyelinating lesions.
Reverse sigmoid type cervical kyphosis with minimal Modic-II-endplate changes at C5/6 and C6/7.
Mild disc degeneration is seen in the cervical spine with decreased signal intensity of the intervertebral discs.
Right preforaminal osteophyte-disc complex is seen at C5/6 with possible impingement of the right-sided C6 nerve root.
The central canal and the neural exit foramina are otherwise patent.
No evidence of any disc herniation or relevant stenosis in the thoracic spine.
Sagittal STIR-MRI images demonstrate bone marrow oedema in the L4- and L5-pedicles, adjacent to the pars interarticularis bilaterally.
Also minimal triangular bone marrow oedema is seen at the posterior superior endplate of L5 vertebra.
Minimal disc bulging is seen at L4/5.
Moderate disc degeneration is noted at L4/S1 with decreased signal intensity of the intervertebral disc at this level. Additional broad-based bulging and central protrusion at L5/S1 without any stenosis or nerve root impingement.
No paravertebral abnormality is identified.
Conclusion:
Normal MRI of the brain. No evidence of chronic demyelinating disease.
Right preforaminal osteophyte-disc complex at C5/6 with impingement of the right-sided C6 nerve root.
Bone marrow oedema involving L4-/L5-pedicles bilaterally, in keeping with early-stage spondylolysis at these two levels.
Moderate disc degeneration and bulging at L4/5 and L5/S1 without any stenosis or nerve root impingement.
