Endometrioma

Findings:
A right-sided multilocular ovarian cyst is seen, measuring approximately 7 x 5.5 x 6 cm. There walls of the cystic lesion are mildly thickened. It elicits T1 hyperintense signal, high signal at fat sat sequences and intermediate to low T2 signal with partial diffusion restriction. Possible fluid-fluid level can be noted on the DC images. No obvious postcontrast enhancement.
An area of low T2 signal scarring and small cystic changes can be seen between the posterior uterine wall and the right ovary.
Numerous follicular cysts are present in the left ovary.
Anteverted anteflexed uterus. Junctional zone not thickened. No myometrial lesions.
The vagina and the urinary bladder are unremarkable.
The visualised bowel is unremarkable. No ascites.
There is no lymphadenopathy.
No bony abnormality is seen.

Impression:
MR images are most likely compatible with right ovarian endometrioma. No obvious malignant features.
