Odontogenic keratocyst/odontoma/Periapical cyst

Findings:
An expansile, fairly well-defined, unilocular lytic lesion is seen in relation to the alveolar margin of the maxillary process left to the midline, bulging into the left nasal cavity. It appears unilocular and erodes the inner and inferior cortex of the left maxilla. It measures approximately 22 x 18 x 22 mm (ML x AP x CC), shows thin surrounding sclerosis and internal soft tissue density. The roots of the left upper canine and first premolar protrude into this cystic lesion. Also, a small metallic foreign body is seen on the floor of the cystic lesion adjacent to the root of the left upper canine.

Area of radiolucency is noted surrounding the root of the left upper third molar.
An unerupted and medially displaced supernumerary tooth is seen located between the right upper first and second premolar teeth.
A radiodense lesion is seen in the right maxillary alveolar ridge, extending to the floor of the right maxillary sinus and surrounding the root of the right upper first molar, with radiodensities similar to dental enamel and dentin.

The visualised mandible is unremarkable.
A small mucous retention cyst is seen in the right sphenoid sinus. The paranasal sinuses and mastoid air cells are otherwise clear.

Impression:
Known cystic lesion in the left upper maxillary alveolar ridge, as described above, likely in keeping with an odontogenic keratocysts (OKC), DDX unilocular ameloblastoma.
Unerupted supernumerary tooth between the right upper first and second premolar teeth.
Radiodense lesion surrounding the root of the right upper first molar, likely in keeping with odontoma.
Periapical cyst surrounding the root of the left upper third molar.
